This notice describes how medical information about you may be used and disclosed by Line of Duty Medical, a division of Elevated Health Augusta, and how you can get access to this information. Please review it carefully.
How we use and disclose your health information
To prepare your opinion. We use the service treatment records, C-file documents, and other medical records you provide to review your history and prepare the independent medical opinion or questionnaire you requested.
At your direction. We disclose your information to your attorney or representative when you ask us to, so it can be used in your VA claim.
For payment and operations. We use limited information to bill for services and to run our practice securely.
As required by law. We may disclose information when required by law or valid legal process.
Other uses and disclosures require your written authorization, which you may revoke at any time.
Your rights
You have the right to:
• Inspect and request a copy of the health information we hold for your matter.
• Request a correction of information you believe is inaccurate.
• Request an accounting of certain disclosures we have made.
• Request restrictions on certain uses and disclosures.
• Request confidential communications through a specific channel.
• Receive a paper copy of this notice on request.
• File a complaint if you believe your privacy rights have been violated.
Our responsibilities
We are required to maintain the privacy and security of your protected health information, to notify you following a breach of unsecured information, and to follow the terms of the notice currently in effect.
Changes to this notice
We may change this notice and the revised notice will apply to information we already hold as well as new information. The current version is always available on this page.
